Social Security Disability, Supplemental Security Income and Optional State

Supplement programs provide monthly income for people with severe

disabilities. SSI provides benefits for those who have low income and limited

resources. SSD (Social Security) is based on one’s work history.

 

If you are unable to go to a Social Security office to apply for benefits, call to

arrange a telephone interview. If a disabled person cannot manage his or her

affairs, a family member, friend or legal guardian may complete the application.

You can shorten the time it takes to complete and process your claim if you

have the following when you apply:

Ø  Social Security number and proof of age

Ø  Names, addresses, and phone numbers of doctors, hospitals,

clinics, and institutions that treated you, and approximate dates of

treatment (include patient numbers, if known)

Ø  A summary of where you worked in the last 15 years

Ø  A copy of your W-2 forms (wage & tax statements), or if you’re

self-employed, your Federal tax returns for the past 2 years

Ø  Dates of any military service

Ø  The claim number for any other check you receive (or expect to)

because of your disability

Ø  The worker’s death certificate and proof of marriage if you are

applying as a disabled widow or widower

Ø  Additional financial assistance may be available through
