BIRTH CERTIFICATE
A copy of your birth certificate can be obtained by written request to the
Bureau of Vital Statistics in the county where you were born. There is
usually a fee for this service. To find the address and phone number of the
Bureau of Vital Statistics in your birth county, look in your local library for
the telephone directory of the city in which you were born. You can also
phone the Bureau of Vital Statistics directly. For the phone number, dial 1,
area code of birth county, and 555-1212 and ask for the Bureau of Vital
Statistics number.

IDENTIFICATION CARD
To get an identification card, you can apply at any driver’s license office.
You must be 12+ years, present your Social Security card and have two
items of identification, one of which must list your full name and date of
birth, and a second supporting document such as:
Ø Birth Certificate/Social Security Card
Ø Passport
Ø School record stating date of birth
Ø Baptismal Certificate
Ø Family Bible Record
Ø Florida Vehicle Registration Certificate
Ø Court Order which includes Legal Name
Ø Voter Registration Card
Ø Marriage License or Certificate
Ø Florida or out-of-state Driver’s License
Ø Military ID Card
Fee for identification card is $3.00
